ABSTRACT

Vaccination for children and adolescents is an effective strategy for preventing and controlling infectious diseases. However, clinical reports showed that vaccination may be associated with epilepsy or seizures, potentially increasing public hesitancy about vaccination, and affecting vaccination coverage. At present, no evidence showed that vaccination could damage the nervous system and cause epilepsy or seizures. In this review, we explore the association of multiple vaccinations in children and adolescents with epilepsy or seizures, and further understand the safety of vaccination, thereby improving vaccine coverage in China.

ABSTRACT

Objective:To study the different effects of pre-pregnancy obesity (PO), excessive gestational weight gain (EGWG), pre-pregnancy obesity combined with excessive gestational weight gain (PO+EGWG) of maternal rats on glucose and lipid metabolism in neonatal offspring, and to explore the possible mechanisms.Methods:Animal models of PO, EGWG and PO+EGWG were established by feeding SD rats with high-fat diets at different periods. Thirty-six SD rats were randomly divided into four groups, with nine rats in each group. The control group had a normal diet before and during pregnancy. The PO group had a high-fat diet before pregnancy and a normal diet during pregnancy. The EGWG group had a normal diet before pregnancy and a high-fat diet during pregnancy. And the PO+EGWG group had a high-fat diet before and during pregnancy. The body weight of maternal rats before and during pregnancy and the birth weight of neonatal rats were recorded. Nine male neonatal rats in each group were selected, fasting blood glucose levels were detected by glucometer, fasting insulin levels were detected by enzyme-linked immunosorbent assay kit, hepatic triglyceride and cholesterol levels were detected by glycerol phosphate oxidase-peroxidase method, hepatic lipid deposition were observed by hematoxylin-eosin staining and oil red O staining. The mRNA levels of hepatic key genes in glucose metabolism pathway IR, IRS, AKT and lipid metabolism FASN, SREBP1c, PPARα were detected by reverse transcription-polymerase chain reaction analyses.Results:The pre-pregnancy weight of maternal rats in high-fat diet group before pregnancy (PO group and PO+EGWG group) was significantly higher than those in normal diet group (control group and EGWG group). The percentage of weight gain of maternal rats in high-fat diet group during pregnancy (EGWG group and PO+EGWG group) was significantly higher than those in normal diet group (control group and PO group) ( P<0.05). The birth weight of neonatal rats in PO group, EGWG group and PO+EGWG group were significantly higher than that in control group ( P<0.05), and the birth weight of neonatal rats in PO+EGWG group was the largest. The fasting glucose, insulin level and insulin resistance index of newborn rats in PO, EGWG and PO+EGWG groups were higher than those in the control group, and the mRNA levels of IR, IRS and AKT were lower than those in the control group, but the differences were not statistically significant ( P>0.05). The hepatic triglyceride and cholesterol contents and mRNA levels of FASN and SREBP1c were higher in the EGWG and PO+EGWG groups than those in the control group, and the mRNA level of PPARα was higher in the PO+EGWG group than in the control and PO groups, with statistically significant differences ( P<0.05). Conclusions:Animal models of PO, EGWG and PO+EGWG were successfully constructed by feeding SD rats with high-fat diets before pregnancy, during pregnancy, before and during pregnancy. PO+EGWG had the most significant effects on the birth weight and glucose and lipid metabolism in neonatal offspring. Compared with EGWG, PO had a relatively significant effect on glucose metabolism in neonatal offspring. And compared with PO, EGWG had a relatively significant effect on lipid metabolism in neonatal offspring. The effects of maternal obesity on glucose and lipid metabolism in neonatal offspring were considered to be related to the expression changes of genes in glucose and lipid metabolism.

ABSTRACT

BACKGROUND:Pneumatic lithotripsy and holmium laser lithotripsy for distal ureteral calculi have their own advantages and disadvantages in efficacy and complications, but both of them are currently better treatments. <br> OBJECTIVE:To compare the efficacy and safety of holmium laser lithotripsy and pneumatic lithotripsy for the treatment of distal ureteral calculi through a meta analysis. <br> METHODS:A online search of China Biological Medicine disk, PubMed, Cochrane Library, Springerlink, Medline, and Embase databases was performed from March 1999 to December 2013. A manual retrieval of relevant journals was also done for clinical trials about holmium laser lithotripsy and pneumatic lithotripsy for treatment of distal ureteral calculi. RevMan 5.1.5 software (Cachrane Library) was used for meta analysis. The fol owing indexes were used to compare the results:mean operative time, early stone free rate, stone migration rate, ureteral perforation rate, postoperative hematuria rate and retaining double-J catheter rate. <br> RESULTS AND CONCLUSION:Four prospective studies (a total of 275 patients) were enrol ed, and there were 140 cases in the group of holmium laser lithotripsy and 133 cases in the pneumatic Lithotripsy group. Holmium laser lithotripsy conveyed significant benefits compared with pneumatic lithotripsy in mean operative time [WMD=-16.38, 95%CI (-20.29,-12.47), P<0.000 01], stone migration incidence [OR=0.25, 95%CI (0.10, 0.63), P=0.003], retaining double-J catheter rate [OR=0.44, 95%CI (0.25, 0.78), P=0.004], while early stone free rate, postoperative hematuria rate and ureteral perforation rate had no significant changes between the two groups. Compared with pneumatic lithotripsy, holmium laser lithotripsy can shorten mean operative time and reduce the stone migration rate. Because of the lack of prospective studies and smal sample size, large-sized randomized control ed studies are urgently needed to provide evidence.

ABSTRACT

This paper proposes a method of automatic ECG analysis and arrhythmia diagnosis for telemonitoring system. A multi-buffer technique is applied to organize dispersive ECG data. Then, according to second order derivatives of ECG signals, peaks of R waves are automatically detected. Finally, real-time heart rhythm is decided by results of R wave detection. Tests prove that the proposed method is precise, efficient and stable to be applied in real-time ECG telemonitoring system.

ABSTRACT

Objective To explore a method for real-time pretreatment and character extraction of epicardial signals and promote the research on the electrophysiological mechanism of atrial fibrillation (AF).Methods Firstly,an analysis was made to get the properties of epicardial signals. Then,a method based on template matching was used to extract atrial activities from epicardial signals. At last,the feature points of atrial activities could be recognized.Results The correctness of recognizing feature points of sinus rhythm and AF were 99.7% and 97.0% respectively. Furthermore,only 1~2 s were cost to compute a 10-second data record with this method.Conclusion The proposed method can extract crucial information about atrium depolarization from epicardial signals and is adequate for online analysis.
